Why insurance usually doesn't cover infusions
Generic ketamine for depression is prescribed off-label, and insurers generally decline to cover infusion treatment. A few angles are still worth trying:
- Superbills. Many clinics give you an itemized receipt to submit for out-of-network reimbursement. Parts of the visit, such as the evaluation or therapy time, are sometimes reimbursed even when the infusion isn't.
- HSA and FSA funds. Ketamine treatment prescribed by a licensed clinician is generally an eligible medical expense.
- Spravato instead. If your diagnosis fits its FDA approval (treatment-resistant depression, or major depression with acute suicidal thoughts), insurance, including Medicare, often covers it. For insured patients it's frequently the cheapest route to ketamine-based treatment. See ketamine vs. Spravato.

What to check when comparing prices
- The full series, not one session. Depression protocols almost always start with about six sessions over two to three weeks. Ask for the all-in price of that series.
- What's included. Does the price include the first evaluation, integration or therapy time, and follow-ups? Two clinics quoting the same infusion price can differ by hundreds of dollars once these are counted.
- Packages. Discounted packages are common and legitimate, but be wary of pressure to prepay for many sessions before your first one. You won't know whether you respond until a few sessions in.
- Maintenance. If treatment works, booster sessions are where the long-term cost lives. Ask what the maintenance schedule and price look like after the first series.
- Financing. Many clinics offer payment plans through medical lenders. Read the interest terms carefully; deferred-interest plans get expensive if you miss the payoff date.
Is cheaper worse?
Not necessarily. Price differences mostly reflect rent, staffing and route of treatment (IM is structurally cheaper than IV) rather than quality of care. What matters more than a $100 difference per session: who screens you, who monitors you during sessions, and how the clinic handles people who don't respond.
